Why GLP-1 for Weight Loss is More Successful with a Doctor
You have probably heard plenty about GLP-1 medications by now. Maybe a friend has lost weight with one. Maybe you have read about semaglutide or tirzepatide online. Maybe you have even visited a website where you can fill out a questionnaire and have medication shipped to your home.
But being curious about GLP-1 weight loss does not mean you are ready for a prescription. It does not even mean medication is necessarily the right choice for you. Sometimes, you just need a place to start.

Weight-loss care can be an opportunity to sit down with a medical provider, discuss what is happening with your body and understand the different options available to you. That conversation may lead to GLP-1 medication. It may lead somewhere else.
Either way, the goal is to make an informed decision about your health rather than simply obtaining a prescription.

“I've Struggled With My Weight for Years”
Maybe you have lost the same 20 pounds more than once. You have changed how you eat. You have exercised. You have counted calories or carbohydrates. You know what you are “supposed” to do, but maintaining the results has been difficult.
This is often where people become interested in medications such as semaglutide or tirzepatide. And there is good research behind that interest.
In the STEP 1 clinical trial published in The New England Journal of Medicine, 1,961 adults with overweight or obesity were studied for 68 weeks. Participants receiving once-weekly semaglutide plus lifestyle intervention lost an average of 14.9% of their body weight, compared with 2.4% among those receiving placebo plus lifestyle intervention.
The SURMOUNT-1 trial studied tirzepatide in 2,539 adults who were obese or overweight and had at least one weight-related complication. Depending on the dose, participants experienced substantial reductions in body weight over 72 weeks.
These medications can be powerful tools for appropriate patients. But that does not mean every person who has struggled with weight should automatically receive one.
A medical consultation gives us an opportunity to look beyond the number on the scale. We can talk about your health history, medications, previous weight-loss attempts, lifestyle, goals and other factors that may be affecting your weight.
Sometimes the most useful first question isn't, “Which GLP-1 should I take?”
It is, “Why am I having such a difficult time losing weight?”
“I'd Really Only Like to Lose 5 – 10 Pounds”
This is an important conversation because wanting to lose weight and being an appropriate candidate for weight-loss medication are not necessarily the same thing. Perhaps you are generally healthy and active, but your weight has slowly increased over the past few years. You would like your clothes to fit the way they used to, feel more comfortable on vacation or get back to a weight where you felt your best.
Does that mean you need a GLP-1 for weight loss? Not necessarily.
The major clinical trials behind GLP-1-based obesity treatments generally studied people who met specific BMI and health criteria. For example, the STEP 1 study mentioned above enrolled adults with a BMI of at least 30, or at least 27 with one or more weight-related conditions.
That does not mean your concern is unimportant. It means it deserves a broader conversation. We may want to talk about nutrition, activity, sleep, stress, body composition, hormones or other factors before deciding whether medication makes sense.
At Premier Integrative, the objective isn't to find a reason to prescribe a GLP-1. It is to help determine which approach makes sense for you.
“My Weight Changed Recently and I Don't Understand Why”
This is one of the strongest reasons to talk with a medical provider rather than immediately ordering weight-loss medication online. If your body has changed and you do not understand why, we want to hear about it.
Perhaps you have gained 15 pounds despite eating roughly the same way you always have. Your energy has dropped. Your sleep is different. Exercise seems harder. Or your weight has gradually shifted as you have moved through your 30s, 40s or 50s.
Weight changes can have many possible contributors. Sleep, stress, activity, diet, medications, hormonal changes and health conditions can all be part of the picture. For a busy executive, parent or business owner, some of those changes may have happened so gradually that they are easy to overlook. Longer workdays replace workouts. Sleep gets shorter. Stress increases. Meals become less predictable.
That context matters. A prescription may address appetite and help with weight loss, but it does not automatically explain why your weight changed. That is one of the differences between comprehensive medical weight-loss care and simply completing an online questionnaire.

“I'm Interested in GLP-1s, but I'm Worried About Side Effects”
That's a reasonable concern. GLP-1-based medications are real medications with potential side effects, contraindications and considerations that should be discussed with a medical provider.
Gastrointestinal symptoms are among the most common side effects. In the SURMOUNT-1 tirzepatide trial, gastrointestinal events were the most frequently reported adverse events and occurred most often while doses were being increased.
The conversation should therefore be about more than whether the medication “works.” We want patients to understand what they are taking, why a particular dose is being considered, what side effects to watch for and when they should contact their provider.
Follow-up matters, too.
Your experience on a medication can change as your dose changes and as you lose weight. Having an ongoing relationship with a medical provider creates an opportunity to discuss symptoms, progress and concerns rather than trying to figure everything out yourself.
“I Don't Know Whether I Want Medication”
You don't have to. There is nothing wrong with walking into a consultation undecided. In fact, that can be a productive place to begin.
You may want to understand how semaglutide and tirzepatide work. You may want to know whether you meet the typical criteria for medical weight management. You may be concerned about side effects, muscle loss, what happens when medication is stopped or whether another approach makes more sense.
Those are all conversations worth having before making a decision. We can discuss what you have tried, what you want to accomplish and what you are comfortable with. Then you can decide how you want to move forward.
Not All GLP-1 Sources Are Equal
The rapid popularity of GLP-1 medications has also created a marketplace that can be difficult for patients to navigate. FDA-approved medications, legitimately compounded medications, counterfeit products and illegally marketed products are not interchangeable categories.
The FDA specifically warns that unapproved GLP-1 products do not undergo the agency's premarket review for safety, effectiveness and quality. The agency recommends that patients obtain prescriptions from medical providers and fill them through state-licensed pharmacies.
There have also been documented problems.
The FDA says it has received reports of dosing errors involving compounded injectable semaglutide, including errors that resulted in hospitalization. The agency has also warned about fraudulent compounded products whose labels falsely identified pharmacies as their source. As of May 31, 2026, FDA had received 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide, although those reports do not by themselves establish that the compounded drug caused the event.
Counterfeiting is another concern. In 2025, the FDA reported seizing counterfeit Ozempic from the U.S. drug supply chain. In one investigation, counterfeit needles were identified and their sterility could not be confirmed, creating a potential infection risk.
The FDA also warns that illegally marketed GLP-1 products sold online could contain too much, too little or none of the stated active ingredient, or potentially contain harmful ingredients.
For patients, the lesson isn't that every compounded medication is inherently unsafe. Compounding has legitimate medical uses. Rather, where your medication comes from, why it is being prescribed and who is overseeing your care matter.
Medical Weight Loss Should Be Medical Care
The convenience of online weight-loss services is easy to understand. Most of us already have busy lives. Adding another appointment to the calendar can feel like one more obligation. But there is value in sitting across from someone who can listen to the whole story.
At Premier Integrative, weight is not the only thing we can discuss during a medical visit. We can talk about your energy, sleep, health history, medications, lifestyle, concerns and what you actually want to accomplish.
For one patient, GLP-1 medication may be an appropriate part of that plan.
For another, losing five pounds may not justify medication.
For someone else, an unexplained change in weight may make us want to look more closely at what is happening before discussing treatment.
And another patient may simply leave the first appointment with better information and more confidence about their choices.
All of those can be successful consultations.
You Don't Need to Arrive With an Answer
There is a tremendous amount of information about GLP-1 weight loss online. Some of it is excellent. Some is oversimplified. And some is designed primarily to sell you something.
